Application Process for New Scholars
The application period for new scholars is open December 20-March 31.
Click the link below and read all of the instructions carefully. PLEASE NOTE: There is an option in the online application for you to “Save and Resume” your work. A link will be sent to you via email for you to return to the application when you are ready to continue. However, any documents uploaded are not saved. Only upload documents when you are ready to submit your entire application. The documents required for this application are as follows:
- Prescription medication form for anyone taking prescription medications during the program: (must be completed and signed by scholar’s doctor): Form MDH-4758-A. A link to the form is provided in the application.
- Three (3) letters of recommendation must be submitted. Instructions regarding letters of recommendation are outlined in the application. New applicants are also required to write a 500-word essay. Your essay can be typed directly into the application or you can cut and paste your essay from a word document directly into the application.
Application for Returning Scholars
